Please note this form is only for use for previously confirmed bookings. If you are not sure if your cat has been reserved a space in cattery please contact your clinic directly by phone.
Section A - General Details

Drop offs and pickups need to be done between 9-12pm and 3-5pm weekdays or Sat 9-12.30pm unless by prior arrangement. We are unable to have cats dropped off or picked up on Sundays or public holidays.

Please note: Cats staying in a Veterinary Hospital Group cattery need to have been vaccinated within the past 12 months. If your cat was vaccinated at another vet clinic please provide proof of vaccination by email or in person

to the day of the booking
4/ Dietary Information
We provide a good quality dry cat food for cats in our cattery. Please note below if you will be supplying your cat’s own food for their stay.
5/ Parasite prevention
All cats boarding at a Veterinary Hospital Group clinic must be up to date with a registered flea preventative. If your cat is not up to date with flea prevention, they will be treated on admission at your expense.Please advise product and last application date:
6/ Emergency Care
If your pet becomes ill during their stay we will always attempt to contact you to update you and discuss any required treatment. If we can not contact you, your pet will be treated as required by our experienced veterinary team at your expense. The Veterinary Hospital Group is happy to organise "standby" payment arrangements for clients going on holidays, in case veterinary treatment is required.
